Patchogue Office Directions

Long Island Expressway to exit 62 south (Nicolls Road south). Nicolls Road south to Sunrise Highway east (27), and then turn south at Route 112 (Medford Avenue). Continue south on Route 112 approximately 7/10 of a mile south of Sunrise Highway (office is north of Montauk Highway - 27A). The Clinical Office, Neurodiagnostics, and Brookhaven MRI are all located in the white office building on the east (left) side of Route 112, on the corner of Columbia Street.
From Long Island Expressway to exit 64 south, which is Medford Avenue (also Route 112). Take Route 112 (Medford Avenue) south crossing over Sunrise Highway (27). Continue approximately 7/10 mile south. The office is located in the white office building on the left (east) side of Route 112, on the corner of Columbia Street.
From Sunrise Highway to exit 53 south - which is Medford Avenue (Route 112). Turn south, drive approximately 7/10 mile south. The office is located in the white office building on the left (east) side of Route 112, on the corner of Columbia Street.
From Montauk Highway Take Montauk Highway (27A) to Route 112. Drive north approximately 2 blocks. The office is located in the white office building on the right (east) side of Route 112, on the corner of Columbia Street.
