A liability action against someone else is not a reason for delaying payment of your bill. Payment of the bill is the responsibilty of the individual who has received treatment.

We cannot release your medical information to your attorney without written authorization. Preferably, this authorization would state the specific purpose and reason for requiring your records, and must also be accompanied by a signed release form from you. Your attorney will be charged for professional services involved in handling this request.

If you are involved in a workmen's compensation case, we require acceptance of financial responsibility from your employer or insurance company prior to your visit.If you are involved in a motor vehicle accident or claim a No-Fault case, we require information about the insurance carrier and verification that it is an active case prior to your visit. You will be required to sign a release of information authorizing us to bill your carrier directly or you will be responsible for the bill for services rendered.
